Dr. Liao likes her patients to be informed consumers and encourages them to learn as much about this procedure as possible.
Fat from ones body can be transplanted to other areas without
causing rejection or allergic reaction. For some, there is an abundant
supply of fat that can be harvested. For others, only a little fat,
but the amount may be sufficient for cosmetic improvement. Small aliquots
could be safely injected to areas where there are defects that need
the volume increased, e.g., depressed acne scars, depressed nasolabial
folds (as a result of aging or softening of lines in the chin area).
Fat reabsorbs initially, but normally 50% is retained and if the transplant
is repeated a few times, then there will be significant improvement
of the areas. Small amounts have to be layered for a greater volume,
this can be done with fat that was harvested from the first time and
frozen then thawed before injection.
Fat harvesting is best done from areas that are resistant to exercise
and dieting, e.g., hip and abdomen. This is done with instruments
that do not traumatize the fat cells too much, as we want the cells
to live and hopefully grow. That is why only small amounts are injected
to allow the fat to be in contact with blood vessels that will nourish
the cell. Fat now has become recognized as a tissue filler, as it
has no cost, and for most people there are ample amounts.

Small globules of fat are injected into tracts that are made under
the tissue which needs to be elevated. However the trauma of extraction,
washing and reinjection, could cause breakdown of these cells, so
minimal trauma is essential to minimize destruction and reabsorption.
Most literature mentioned that 50% should remain after 6 months. So
if there is further reinjection to the same areas, especially with
layering of the fat, the areas should fill up to a certain degree.
Many surgeons have seen satisfactory results after 3-4 years post
injection.
The area to be corrected has to have the skin undermined and tracts
made under the skin to accommodate the fat cells without too much
pressure. This area will be badly bruised as the creation of tracts
will result in small blood vessels being traumatized. The area is
also over-corrected with fat to allow for over 50% reabsorbation,
so patients will be swollen and bruised, most likely it will last
anywhere from a few days to 2 weeks.
Sterility of the fat retrieval and injection is always maintained
so the infection rate is small, but the chances are always there especially
when working around the mouth.
In one case reported by Dr. Teimouiran when fat was injected into
the glabella frown-line (forehead, between the eyebrows) one eye had
pain and loss of vision. This is rare and only happens at the glabella
area as the fat went into the blood vessel of the eye.
Calcification has also been reported on fat transplants for breast
augmentation. Because of these calcifications, breasts will not be
augmented with fat in our office, because it causes confusion when
mammograms are done to know if these are related with tumor changes
or not.
Dr. Liao will store fat harvested for future injections for you in her freezer, However, we cannot be liable for any power failures or unforseen circumstances that may render the fat not usable. If such unforseen circumstances should arise, then there will be additional charges for further fat harvesting.
